Please complete all of the following requirements before arriving on campus.

1. Non-degree student and auditor immunization requirements

All students and auditors who will be present on campus must submit proof of required vaccinations and complete the Tuberculosis Screening Questionnaire. If you have previously submitted these during an earlier term, you do NOT need to resubmit.

Please note: Failure to complete the requirements by the deadline will result in a registration hold that will prohibit you from registering for future courses.听

2. How to complete your immunization requirements

Visit your to complete your immunization requirements:听

  • Log in using your CU IdentiKey and password听
  • Select 鈥淕et Started鈥 under Student Health Requirements听
  • Select option 鈥1. Enter Vaccinations鈥 and provide two valid MMR** dose dates/titer information or utilize VeriVax听
  • Select option 鈥2. Upload Proof鈥 to provide supporting documentation*听听
  • Select option 鈥3. Complete Forms鈥 to complete the TB Screening Questionnaire and any follow-up tasks听

*All documents submitted to the portal must include your full name and date of birth听

**Note: Students born before Jan. 1, 1957 are exempt from the MMR requirement and are only required to complete the Tuberculosis Questionnaire.听听
